The sheet is one list for the whole group. For one person's preferences, use the Secret Santa questionnaire. Choose a budget everyone can manage before opening sign-up, then send an invitation with a clear deadline. What to Put on a Secret Santa Sign Up Sheet

Ask for the name each person uses in the group. When two people have the same first name, add a surname or initial before drawing so their links are easy to tell apart. One person gets one row; whole households need a different setup if they are giving as a single unit.

The gift-idea column is optional and intended for a short preference someone is comfortable sharing. Keep home addresses, clothing sizes and allergy information off a sheet displayed at work. A private message or individual questionnaire is a better place for details that only the organizer or gift buyer needs.

Before the draw, check that every name is an opt-in, nobody appears twice and no late reply was missed. After drawing, send each link to its own person. The sign-up sheet lists who is participating; it never needs to list who drew whom.
