There are two ways to use it. Print a blank copy for each person, or post the questions in the group chat: with nothing filled in, Copy answers as text copies the questions, ready for everyone to answer. Once the answers are in, draw names and send each Santa their private link.
A questionnaire is one person answering questions about what they like; a wish list names the things themselves, and the two work well together. For an office exchange, the Secret Santa rules for work cover the budget and the timing. For a family Secret Santa, the Family set asks about sizes and something the whole household could share.
Questions for a Secret Santa Questionnaire
Each set here has 10 or 11 questions, which fit on one printed page with room for a few of your own and take a few minutes to answer. The useful ones tell a Santa where to start and what to skip:

Most questions are open, so people answer in their own words; a few are yes or no, where that settles it, as with scented things or alcohol. Add your own under Choose the questions; it goes on the sheet with the rest.
Secret Santa Questionnaire for Coworkers
The questionnaire above starts with the Coworkers set: short, work-safe questions about snacks, coffee, desks and lunch. Teachers and school staff can use it as it is. What to leave off a work questionnaire:
- Clothing sizes: nobody should have to write them down for a colleague.
- Alcohol, unless you know the whole team is comfortable with it.
- Home addresses: if gifts are posted, the organizer collects addresses privately.
- Questions about partners, children or religion.
- Anything about bodies, such as skincare or fitness goals.
Before you print, open Group name, budget and date and fill them in, so they appear at the top of every copy and nobody has to ask.
Secret Santa Questionnaire for Adults
For friends, neighbors or a mixed group of adults, the Adults set relaxes a little. Instead of the desk and lunch questions, it asks:
- Do you drink alcohol? If so, what do you like?
- Sizes, if clothes are welcome
- Something for your home or kitchen you would use
- An outing you would enjoy: a class, a show, a meal out
- Books, music, shows or games you are into
- Pampering gifts: yes please, or not for me?
The Family set drops the alcohol and pampering questions and adds something you need but would never buy yourself, something your whole household could enjoy, and whether homemade gifts are welcome.
Secret Santa Questionnaire for Kids
Children need fewer words and more room to write. The Kids set asks:
- How old are you?
- Favorite character, show or movie
- Favorite animal
- Favorite toys or games
- Do you like drawing, building or making things?
- Favorite sport or thing to do outside
- Something you already have lots of
The allergy question says a grown-up can fill it in. For a class, two per page saves paper. When you draw the names, tick the box for groups that include children under 13: their cards will never show ads or shopping links.
Secret Santa Questionnaire: This or That
Quick pairs are easier to answer than open questions, and fun to read out at the exchange. On paper, each person circles one of each pair; on screen, they tap one. What the answers tell a Santa:

The Kids set has its own pairs: cats or dogs, drawing or building, indoors or outdoors, books or games, sweet or sour, stickers or puzzles.

The blank coworker questionnaire is a free PDF in two paper sizes: US Letter and A4. Each is one page, with a QR code at the foot that opens this page, for anyone who would rather fill it in on screen.
For the Adults, Family or Kids set, your own questions, or two copies on one sheet, set them up at the top of this page and press Print blank or Print with answers. To keep that as a PDF, choose Save as PDF as the printer, with Letter or A4 as the paper size. Only the questionnaire prints, with the group name, budget and date at the top and the site's address at the foot.
2 per page puts two copies on one sheet, top and bottom, to cut in half. For a Word or Google Docs version, press Copy answers as text with nothing filled in and paste the questions into a new document; the same list works as the questions of a Google Form.
